Does anyone remember a challenge community that had achievement badges for things? Like "five fics in a row" or "five different mediums" and things like that?

Multiple communities have done that. Some bingo or challenge communities still do. allbingo has a page for Bingo and Achievement descriptions, if you want a ton of examples for challenge targets. Some are probably unique, others were gathered from common practices at the time of posting. This community hosts at least one fest per month, occasionally more. Fest hosts are free to make whatever banners / badges they wish to offer.
